Dave on Does having your running shoe fit for your arch type really matter?

In a recent peer reviewed journal the following well done study came
out entitled “Injury reduction effectiveness of assigning running shoes based
on plantar shape in marine corps basic training”. The study found no statistically significant
reduction in injuries in a group who were given a motion control or cushion shoe based on whether
they pronated or supinated versus a group that got a general stability shoe
regardless. These findings were echoed
in recent studies done by the Air Force and Army. Essentially
the study says fitting a shoe
based on only your arch height does not impact the potential for
injury. This study used a control group, the neutral shoe group and was
large enough, over 5000 participants if you combine all the studies.

This conflicts with a study done in 2001 at Ft Hood which
had almost 10,000 participants. This
study found a decrease in serious injuries when footwear was fitted to foot
type. I am in process of reviewing this
study for work in its entirety but have to wait to see if the military
will send it to me, but so far some issues were that this study looked
at fit
for arch height and foot
flexibility, there does not seem to be a control group, the way adverse
outcomes were tracked In the marine corp study there were follow ups
whereas in the Fort Hood study data came from review of treatment
records from the physical therapy clinic at Fort Hood before and after
the study was completed and most importantly whether
the population followed though with the shoe recommendations as the
participants had to purchase them (it is estimated
that less than 15% did).

Based on the above, it seems that relying solely on fitting
a shoe to your arch may not be appropriate to prevent injuries. One should consider proper training most
specifically with regard to training intensity increases. Recently I have read
some research about the the use of eccentric contraction
exercises to the muscles that move the hip and knee (one such study showed significant
pain and functional improvement in runner's knee, patellofemoral pain syndrome, in using eccentric exercises for
the hip versus the knee)

Also if anyone would like a copy of the above mentioned
articles, please let me know.

Dave
